Please click on the link below to download a copy of our Apostille Service Order Form. Please complete the form in its entirety (5 pages), please make sure that you add up the total amount due, and please enclose this form with the documents that you are sending. Please place a note (like a "Post-It" note) on all of your documents to label what country each document is going to. Please enclose a printout of your receipt from Pay Pal if you have paid with your credit card. Please enclose your Money Order for the appropriate amount. We will email you a Pay Pal invoice for the total due if you have indicated on the form that you want us to send you an invoice once we receive the documents. If paying with credit card, use the Pay Pal Buttons below to process your order, once you have added up the total due using the Apostille Service Order Form.
