Apostille Service Order Form

Please click on the link below to download a copy of our Apostille Service Order Form. Please complete the form in its entirety. We can now authenticate all Notaries Public in California whose commissions are valid. You can use the Pay Pal button above to pay for your Apostille Service if you know your total charges. Please place a note (like a "Post-It" note) on all of your documents to label what country each document is going to. Please enclose a printout of your receipt from Pay Pal if you have paid with your credit card. Please enclose your U.S. Postal Money Order (or approved Business Check for businesses which have called us for our approval) for the appropriate amount. We can also email you a Pay Pal invoice for the total due if you have indicated on the form that you want us to send you an invoice once we receive the documents.

To pay with through Pay Pal, first print out the Apostille Order Form for California above. Next, click the Pay Now button above- this will take you to the Pay Pal website where we accept payments. Next, put in the amount of your payment, and click "update total", and the total you are going to pay will appear- please make sure that it is the correct amount. Next, you can sign into your Pay Pal account if you have one, or if you don't, click "continue without signing into Pay Pal" and fill out all the requested Credit Card information. Next you will be asked to review all of your information before the payment becomes final. When you submit all the information, you will see a receipt with a 16-digit confirmation number (which will also be sent to your email address).
